- MHAC Tryout Registration Form Please pre-register for 12U and 14U teams by November 9th and 16U teams by November 16th.
- Obtain player CEVA Membership
- Complete the CEVA Health Release Waiver and the CEVA Concussion/Cardiac Form
- $20 Tryout Fee (non-refundable-bring to tryouts)
